Southwest Catholic Health Network (SCHN) was established in 1985 as a not-for-profit managed care corporation sponsored by Catholic Healthcare West and Carondelet Health Network. SCHN, doing business as Mercy Care Plan, has managed health care for people in Arizona for more than 20 years, including those with special needs.

In support of this mission, Mercy Care Plan provides health care coverage for members enrolled in AHCCCS, the state's Medicaid program, including:
  • Families
  • Children
  • Single persons
  • Couples without children
  • Elderly and physically disabled
  • Medicare cost sharing members
  • Individuals with developmental disabilities
As of January 2006, Mercy Care Plan is pleased to offer a Medicare Advantage Prescription Drug Plan, called Mercy Care Advantage, for Mercy Care Plan members that are eligible for both Medicare and Medicaid. Mercy Care Advantage has a contract with the Centers for Medicare and Medicaid Services (CMS). This contract renews each year. At the end of each year, the contract is reviewed and either Mercy Care Advantage or CMS can decide to end it. If this happens, you will receive 90 days advance written notice. Additionally, Mercy Care Plan provides care for members enrolled in Mercy Healthcare Group, a state-funded program.

Services are funded primarily under contract with AHCCCS and the Centers for Medicare and Medicaid Services(CMS). Mercy Care Plan/Mercy Care Advantage follow federal and state laws that apply under the contracts with AHCCCS and CMS.
