Contracting Information

The Provider Services Department receives many requests from potential providers to contract with Mercy Care Plan/Mercy Care Advantage. Some of the factors considered in any potential contracting decision are:
  • Geographic network needs based on Federal and State requirements
  • Existing provider coverage in the targeted area
  • Membership requirements for a particular specialty
  • Member accessibility
If you are interested in a contract with us, please print and complete the Initial Request Form and submit, along with a non-binding letter of intent to:

Mercy Care Plan/Mercy Care Advantage
Attn: Provider Services Department
4350 E. Cotton Center Blvd.
Bldg D
Phoenix, Arizona 85040

Once a letter of intent is received, the next step in the contracting process is a comparison of our membership needs to current network availability. The following information is obtained:
  • Board certification
  • Languages spoken
  • Office locations
  • Network assessments (only for providers not joining a contracted group)
If the assessment indicates a need for an additional provider, a comprehensive site visit and medical chart review will be conducted. In addition, AHCCCS/Mercy Care Plan/Mercy Care Advantage participation requirements, policies and provider obligations will be discussed.

Credentialing is initiated once the site audit is approved. Mercy Care Plan/Mercy Care Advantage credentials providers according to nationally recognized standards that serve as the model for minimum credentialing requirements.
