Fill in the information requested below to find a health care provider in your
area.
INSTRUCTIONS:
Search by last name. Enter the last name of the doctor, dentist
or hospital you are looking for.
Search by location. Enter a ZIP code. If you would like to search
in a general area, enter the ZIP code and select how many miles around the ZIP code
you would like to search. You can also search by city.
Search by specialty. Use the drop down box to find a doctor or
dentist by specialty.
Please note: Optometrists included on the Vision Service Plan (VSP) list are contracted directly with VSP to provide routine eye care for Mercy Care Plan members.
