At Mercy Care Long Term Care, we believe your primary care provider (PCP) is the key to keeping your condition stable and keeping you out of the hospital or a nursing home for as long as possible. That’s why you need to see your PCP regularly.

We ask that you see your PCP every three months to go over any changes in your health and review all medications. At each home visit, your case manager will bring you a form and will go over any symptoms you have had in the last three months. Your case manager also will help you schedule an appointment with your PCP.

When your PCP is familiar with you, your condition and your health care needs, he or she is more likely to notice changes in your condition and be able to take proactive steps in your treatment.
