Arizona State Hospital

All Mercy Care Plan Long Term Care members that reside at Arizona State Hospital (AzSH) will be assigned a Maricopa Integrated Health Systems’ (MIHS) PCP. Further, all medical services for MCP members residing in the AzSH that are not provided by AzSH will be provided by MIHS Clinics and or Medical Center.

For all Mercy Care Long Term Care members discharging from Arizona State Hospital (AzSH), the Mercy Care Plan Long Term Care case manager and behavioral health placement coordinator will assist in ensuring that care continues without interruption at time of discharge. This, includes ensuring that for members that are diabetic and being discharged from AzSH, they are issued the same brand and model of both glucometers and supplies they were trained to use while residing at AzSH.
