Specialty Medications
For Mercy Care Long Term Care members to obtain medications on the Specialty Injectable Medications Drug List, the provider must:
- Complete the Specialty Medication Authorization Form.
- Fax the form to CuraScript™ at (888) 773-7386.
Completion and submission of the Specialty Medication Authorization Form will initiate the prior authorization process for drugs on the list. Additional information may be requested, such as pertinent medical records.
Once approved, CuraScript™ will deliver the medication to the location identified on the form (e.g., the provider's office or member's home) within 48 hours.
