Your DD Member Representative

As a member or the guardian of a member enrolled in the DD program, you have access to Mercy Care Plan’s DD member representative. You can reach the representative calling DD Member Services at (602) 238-9028.

The member representative is your primary contact at Mercy Care Plan. The representative can help you with things like:

  • Getting a prior authorization
  • Finding a provider
  • Communicating with the state’s DDD office
  • Coordinating care
  • Getting behavioral health services
