Arizona State Hospital

All Mercy Care Plan members that reside at Arizona State Hospital (AzSH) will be assigned a Maricopa Integrated Health Systems’ (MIHS) PCP. Further, all medical services for MCP members residing in the AzSH who require medical services that are not provided by AzSH will be provided by MIHS Clinics and or Medical Center.

For all MCP members discharging from Arizona State Hospital (AzSH), the Mercy Care Plan behavioral coordinator will assist in ensuring that care continues without interruption at time of discharge. This includes ensuring that for members that are diabetic and being discharged from AzSH, they are issued the same brand and model of both glucometers and supplies they were trained to use while residing at AzSH.

When a member’s mental health status renders them incapable or unwilling to manage their medical condition and the member has a skilled medical need, Mercy Care Plan will arrange ongoing medically necessary nursing services in collaboration with the member’s PCP.
