Caring for Patients with Chronic Illnesses

Patients who have chronic illnesses often need additional support. At Mercy Care, we work with you to provide that support to patients who have been diagnosed with one of the following:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Congestive Heart Failure
  • Diabetes

Our disease management programs help members reduce the frequency and severity of chronic illness episodes by improving their health status and helping them manage their disease. Our programs have been developed using nationally recognized evidence-based guidelines.
