Effective 01/01/2013, Mercy Care Plan/Mercy Care Advantage will transition from Express Scripts, Inc. to CVS Caremark. There is no change to the current network, except the addition of Walgreen’s pharmacies. Note, there are a new Prior Authorization (PA) FAX and PHONE numbers. Coverage Determination Requests for Mercy Care Advantage Members only must be submitted to CVS Caremark. Please submit requests to CVS Caremark via fax at 1-855-230-5544 or by calling 1-855-651-4849.
