Specialty Medications
In order for Mercy Care Plan patients to obtain medications on the Specialty Medications Drug List, the provider will need to:
- Complete the Specialty Medication Authorization Form.
- Fax the form to Mercy Care Plan at 1-800-854-7614.
Completion and submission of the Specialty Medication Authorization Form will initiate the prior authorization process for drugs on the list. Additional information may be requested, such as pertinent medical records. Once approved, CuraScriptTM will provide the medication to the location identified on the form (e.g. the provider's office or member's home) within 48 hours.
