Specialty Medications

In order for Mercy Care Plan patients to obtain medications on the Specialty Drug List, the provider will need to:

  • Call (602) 263-3000 (Toll Free (800) 624-3879) to initiate prior authorization for the requested specialty medication.


    • OR
  • Complete the applicable Pharmacy Prior Authorization form and fax to (800) 854-7614.

Specialty medications can be delivered to the provider’s office, member’s home, or other location as requested.
